LanceSoft is seeking a travel nurse RN Home Health for a travel nursing job in Jackson, Michigan.

Start Date: August 12, 2025(can be moved as per candidate's availability)
Schedule: Monday–Friday, 8:00 AM – 4:30 PM
Contract Duration: 13 weeks

 

Position Overview:

Seeking a qualified and compassionate Registered Nurse (RN) for a 13-week Home Health contract assignment in Jackson, MI. This position involves providing direct care to patients in their homes, utilizing clinical skills to deliver comprehensive nursing services and coordinate care with allied health professionals. Candidates must be able to operate independently and demonstrate strong clinical judgment and communication skills.

 

Responsibilities

• Provide skilled nursing care to patients in the home setting.
• Conduct in-home assessments and develop individualized care plans.
• Coordinate with physical therapists, occupational therapists, social workers, and other healthcare professionals to deliver comprehensive home health care.
• Document patient care accurately using appropriate systems.
• Monitor patient conditions, administer medications, and manage treatments as prescribed.
• Educate patients and families on treatment plans and health maintenance.
• Track mileage from home HF site to patient homes for reimbursement (67 cents per mile via Kronos).
• Ensure compliance with all agency, state, and federal regulations.

Requirements

 

Licensure & Certifications:

• Current RN license issued by the State of Michigan.
• Current BLS certification (AHA required).
• Valid Michigan driver’s license with a clean driving record.
• Reliable transportation and a working telephone.

Experience:

• Minimum of 2 years of home health experience required.
• Strong communication, interpersonal, and organizational skills.

Preferred Qualifications

• Experience with IV therapy, OASIS documentation, and Epic EMR.
• Background in acute care settings.

Additional Information

• Mileage is reimbursable at the rate of $0.67 per mile.
• No weekend or on-call coverage currently required.
